Intussusception Causing Painless Hematochezia in an Adult

چکیده
A 63-year-old woman was hospitalized after 2 episodes of mucoid, maroon-colored stools. She denied any prior or current episodes of abdominal pain, vomiting, fever, or recent travel. She was hemodynamically stable without significant drop in hematocrit. Colonoscopy revealed a 5-cm polypoid mass in the ascending colon with ulceration and fat eroding through the mucosa (Figure 1). The colonicsegment proximal to the mass could not be visualized on colonoscopy, but abdominal computed tomography (CT) showed a 5 x 3 x 4-cm fat density mass at the hepatic flexure causing colocolic intussusception of the ascending colon into the proximal transverse colon. Axial CT demonstrated the classical “target” sign and coronal view demonstrated a “sausage-shaped” mass (Figure 2). Initially, the patient refused surgical intervention but subsequently agreed, as she had recurrent rectal bleeding. On laparotomy, the ascending colonic segment was found gangrenous; therefore, a right hemicolectomy with ileocolic anastomosis was performed. Histolopathology confirmed a benign submucosal lipoma (Figure 3) and normal lymph nodes. Postoperatively the patient recovered well.

Jejunojejunal Lipoma Causing Intussusception
Jejunojejunal intussusception and jejunojejunal lipomas are both very rare clinical entities. The present case report describes this event in an adult patient with severe episodes of hematochezia and colicky upper abdominal pain. The diagnosis was not made preoperatively, as none of the endoscopic findings were pathognomonic and, furthermore, CT scans could not rule out malignancy. Exploratory ...

Adult intussusception is clinically rare. We report a case with colon intussusception caused by a pedunculated polyp of the sigmoid colon in a 54-year-old male. Abdominal ultrasonographic screening for hematochezia showed intussusception in the sigmoid colon, demonstrating a multiple concentric ring sign and a mobile leading colon polyp. Histological examination of a biopsy specimen of the poly...
متن کاملEnterogenous cyst of the small bowel causing intussusception in an adult: Case report and review of literature
Small bowel intussusception is a rare congenital disease entity, mostly manifesting before the age of two with abdominal pain and acute intestinal obstruction. In adults, colon intussusception is a rare event causing approximately 1% of all acute intestinal obstructions and they can occur idiopathically in adults or due to an intraluminal tumor mass. We report a case of an enterogenous cyst cau...
متن کاملSmall bowel intussusception causing a postoperative bowel obstruction following laparoscopic low anterior resection in an adult.
Adult intussusception usually presents with nonspecific symptoms such as abdominal pain, bloating, nausea, vomiting, and a change in bowel habits. Although postoperative intussusception has been described in the pediatric population, there has been little description of it in the adult population. Postoperative intussusception has unique challenges, as hydrostatic reduction may compromise bowel...

Hematochezia as an acute and chronic lower gastrointestinal bleeding could be caused by diverticulosis, angiodysplasia, neoplasm, perianal disorders, Meckel's diverticulum, colitis (infectious and non-infectious) intussusception, and many others. Lower gastrointestinal bleeding mostly occurs in older age. Mortality caused by acute and chronic lower gastrointestinal bleeding is very high. On the...
